Body contouring surgery is used to enhance and improve the appearance of ones body, by reshaping it to attain a more normal body appearance. Following major weight loss, either through dieting and exercise or bariatric surgery, the skin usually is lacking in elasticity and hence cannot conform to the reduced size of the body. This results in saggy, heavy, loose skin, which hangs in problematic folds. For instance the upper arms may appear loose and saggy, the abdomen may result in an apron like overhang, or lastly there may be hanging pockets of skin around the groin, buttocks and thighs. This may not only be upsetting for the individual, hence affecting their self esteem, however may contribute to the development of health problems. The problems which can be caused by heavy and excess skin include personal hygiene issues, skin thinning, skin tearing, back and shoulder aches and pains, poor posture, infections and chronic rashes within the skin folds, as well as difficulty in exercising and walking.

Body contouring surgery is aimed to improve various body areas, such as the upper torso and arms, the waist, abdomen, buttocks and thighs. It is important to note that such a surgery is not a weight loss alternative nor a method of fat reduction, however, should be considered as final phase of your weight loss experience. In addition, liposuction alone should not be considered as the sole option for contouring the body, as liposuction does not achieve skin tightening, and the issue is excess skin not fat. Body contouring surgery is commonly carried out in stages, and Dr Lim will develop a surgical plan that is inclusive of your current condition and desired goals.

Several surgical techniques can be performed to reshape the body by removing excessive amounts of skin and correcting the remaining skin that is left. These techniques include a body lift, which is also known as a truncoplasty or Belt Lipectomy with or without liposuction, an upper arm lift or Brachioplasty, and a medial thigh lift.

Body Lift (Truncoplasty; Belt Lipectomy)

bodylift4

A body lift procedure entails removing the belt of excess skin and fat on the lower abdomen, waist, hips, upper buttocks and outer upper thighs, which can be carried out in one operation or in several stages. The extent of fat and skin to be removed will vary from each patient and may be estimated prior to the surgery. The circumferential incision of a body lift is carefully placed so that can be hidden below the underwear line. Body contouring surgery is performed over several hours, however it is important to note that such a long period of time in surgery is not suitable for every patient. Dr Lim will advise you as to how much skin can be removed safely, keeping in mind your personal objectives.

The technique used to reduce the lower abdomen is similar to an abdominoplasty or tummy tuck, where the existing navel is repositioned, the excess skin that exists on the lower abdomen is removed, and the skin of the upper abdomen is pulled down and joined to the lower fold on the abdomen. The excess skin on the back is usually removed as well, and the remaining skin is tightened. Extra skin and fat on the sides of the hips are also removed, in which the skin over the outer sides of the thighs is tightened, leaving a scar around the hip. In order to lift and tighten the buttocks and thighs, excess skin and fat is also removed, and the scar is placed on the upper part of the buttocks, which dips into the cleft, thus the circle of lifted skin is completed. In some cases in order to enhance the shape of the treated area, liposuction can also be used within the body lift procedure. A buttock flap may also be used to enhance the size and shape of the buttock as these have usually lost its volume and shape.

Upper Arm Lift or Brachioplasty

In an upper arm lift the excess skin that hangs from the under-surface of the upper arm is removed. The amount of skin removed varies and is dependent on the patient. The incision from the underarm area extends along the inside of the upper arm, starting from the armpit to the elbow, so that the scar is hidden when the arms are positioned close to the body. It is important to note that healing around the arm area is quite unpredictable and may result in the scar being thickened or widened. However, the results of upper arm contouring are visible almost instantly, in which tighter and smoother contours are evident on the arms, and the quality of the skin is dramatically improved.

Medial Thigh Lift

A medial thigh lift reshapes the upper inner thighs by removing the excess skin and fat. This is achieved by creating an incision that begins at the fold where groin and thigh meet, and extends along the inner part of the thigh towards the knee, where it is under controlled tension that the excess skin is removed. As a result, the skin is tightened and the contour of the inner thigh is greatly improved

Did you know - Natural substances such as collagen, elastin and hyaluronic acid, that provide the skin with structure and volume, decrease with age. This, together with environmental factors such as exposure to sunlight, smoking and stress all contribute to how our skin ages. The lips in particular have an extremely thin outer layer, which becomes progressively thinner as the body ages. Over time the corners of the mouth can begin to droop (mouth frown), the v-shaped area of the upper lip known as Cupids Bow begins to flatten out and our lips lose their youthful fullness. Vertical lip lines can also begin to appear, causing your lipstick to bleed.

Breast Reconstruction after a Mastectomy

Posted on 19 May, 2013

As you would have seen recently in the news, Angelina Jolie underwent a double mastectomy and a breast reconstruction to prevent her from developing breast cancer. This reduced her risk of developing the disease drastically.

Dr Lim often works with breast cancer survivors and patients at risk of developing breast cancer following mastectomy. This type of breast reconstruction work is very rewarding and an area that Dr Lim is very skilled in.

As you can see from the pictures below, the results are amazing, and gives patients the ability to return to their full form by undergoing this type of procedure.

This patient is a 39 year old mother of 4 children who underwent genetic testing for breast cancer following her mother and grandmother suffered from the disease. She decided to have a double mastectomy to reduce her risk of the disease and the photos above show her final result after Dr Lim worked his magic and after Sydney Permanent Makeup Centre tatooed the remainder of her areola. The patient was thrilled with the results, and went on to have another baby following this.

Administration of Cosmetic Injectables (Doctor versus Nurse)

Posted on 13 May, 2013

The Australian Society of Plastic Surgeons have released strict guidelines for the administration of anti-wrinkle injections.

The guidelines explain "It may be lawfully administered by either the doctor, or a nurse acting under the supervision and instruction of a doctor, and in accordance with a written order provided by the doctor"

Patients need to be aware of this when selecting a facility to undergo these cosmetic injections. It is possible that something could go wrong with any procedure, including injectables. Therefore, the guidelines state that "The doctor should be immediately contactable in order to respond to a nurse or patient’s concern in a timely manner."

It is quite common nowadays for Beauty Salons, and Laser hair removal facilities to have a nurse injector whom administers anti-wrinkle and filler injectables. However, under these guidelines, these premises need to ensure they meet the criteria set out for them. "The premises must be properly equipped to deal with anaesthetic toxicity effects and potentially life threatening anaphylactic reactions. The premises must provide facilities and procedures for all healthcare workers to adhere to infection control principles, including safe injection practices and aseptic technique for the preparation and administration of all injectable medications."

Here at Dr Lim's practice, all of our cosmetic injections are administered by an approved doctor (General Practitioner and trained injector) Dr Moulos. Patients can feel at ease knowing that a Doctor is treating them in a approved doctors facility, should anything go wrong.

Patients need to be more vigilent with their health and do more research into the required qualifications and guidelines by Doctors and healthcare workers before undergoing any treatment.

If you would like to book an appointment with Dr Moulos, please contact Dr Lim's rooms on 9893 9388.

A Natural Beauty Transformation

Posted on 30 Apr, 2013

Previously in the world of celebrities, red carpets and photo shoots, you would notice that the models and actresses seemed to have an ‘overdone’ look in terms of injectables (fillers and muscle relaxers).  As seen in this picture below, many people are frightened that they may end up looking ‘overdone’ if they undergo treatment and therefore decide not to take the step.

However, as seen in the recent SMH article, the new trend is to go ‘lighter’ for a more natural look.  Celebrities such as Nicole Kidman and Gwyneth Paltrow have toned down there injectables to achieve this more natural look.

Muscle relaxers is still the most popular cosmetic clinic treatment according to the Cosmetics Physicians Society of Australia (CPSA), however “the frozen –into-oblivion face is no longer desired”, “people do not want to look too done”.

Here at Dr Lim’s practice, we always aim to achieve a more natural result. Dr Moulos has perfected his art of administering injectables to attain a more youthful look without going overboard. Below are some of our before and after photos to show you these successful outcomes.

If you were previously scared to come in for an injectable treatment, you may now feel more relaxed after seeing some of Dr Moulos’ tremendous results.

Also with mother’s day just around the corner, it would be a great idea to purchase a gift certificate to our practice instead of another bottle of perfume or the like!! I’m sure your mum would appreciate the more youthful confidence that goes along with this type of treatment.

Breast Implants

Breast Augmentation – Do Women Prefer Round or Tear Shaped Implants?

A recent study from the University British Columbia in Vancouver looked at the differences in patient satisfaction when it came to having round or tear shaped breast implants.

A study published in the March 2013 volume of Plastic and Reconstructive Surgery examined patients who underwent breast augmentation, approximately half with round implants and half with tear shaped implants. The study confirmed that patients had no difference in overall satisfaction between round and tear shaped implants

The study looked at 65 women who had round implants and 63 women who had tear shaped implants. The patients were administered a questionare between 1-4 years post surgery and it looked at general satisfaction, satisfaction with care, satisfaction with overall outcome, as well as psychosocial, sexual and physical well-being.

Women who had surgery with tear shaped implants had a satisfaction score of about 77 percent out of 100, whereas women who had round implants placed had a 75 percent satisfaction score. The study confirmed that there was very little difference in satisfaction across the board, except that women with tear shaped implants self-rated their breasts as firmer than the women who had received round implants.

Dr Lim will assess each patient indivdually and discuss whether or not round or anatomical (tear shaped) implants would be best suited to your body. This really does depend alot on the size, shape and amount of skin and tissue the breasts have.

Either way, patients with round or tear shaped implants, (as you can see from this study and from our own patient satisfaction) are very happy with their end result. However, if you have any questions or would like to book an appointment with Dr Lim to discuss breast augmentation please contact our rooms on 9893 9388 or email us at drlim@drjakelim.com.au and our staff will be more than willing to assist you.
